This Disclosure Statement has been prepared in accordance with the Turkish Personal Data Protection Law No. 6698 (“KVKK”) in order to inform patients and patient relatives regarding the processing of their personal data by Dr. Serdar Saraç, operating at Fahrettin Kerim Gökay Cad. 26/3 Hocaalioğlu Apt. (Minibüs Cad. Sahra-i Cedit Bus Stop) Göztepe – Kadıköy / Istanbul.
In the event that you apply to us, undergo an examination, create an appointment, or receive healthcare services, your identity information, contact information, health data, appointment and treatment information, payment and billing information, and other information provided by you may be processed. Identity and contact information of patient relatives may also be processed for the purpose of carrying out communication and information processes.
Health data is considered special category personal data under the KVKK and is processed solely for the purposes of carrying out diagnosis, treatment and care services, planning and management of healthcare services, fulfillment of legal obligations, and ensuring patient safety.
Your personal data is processed based on the legal grounds of your explicit consent, necessity for the provision of healthcare services, fulfillment of legal obligations, and the legitimate interest of the data controller. Special category personal data is processed within the scope of the conditions stipulated under Article 6 of the KVKK.
Your processed personal data may be transferred, limited to the purposes stated above, to authorized public institutions and organizations, accounting and financial consultancy service providers, information technology and archiving service providers, and legal advisors where necessary. Apart from this, your personal data is not shared with third parties.
Your personal data is retained for the retention periods stipulated under applicable legislation and for the duration required by the service relationship. Upon expiration of the retention period, your personal data is deleted, destroyed, or anonymized.
Dr. Serdar Saraç takes the necessary technical and administrative measures to prevent unlawful processing of your personal data, to prevent unauthorized access, and to ensure secure storage of data.
Pursuant to Article 11 of the KVKK, you have the right to learn whether your personal data is processed, to request information if it has been processed, to learn the purpose of processing and whether it is used in accordance with its purpose, to know the third parties to whom it is transferred domestically or abroad, to request correction if it is incomplete or incorrectly processed, to request deletion or destruction, and to claim compensation in case you suffer damage due to unlawful processing.
You may submit your requests regarding these rights in writing or via e-mail to info@serdarsarac.com. Your applications will be concluded within the periods stipulated under the KVKK.
